Question- what are the urls that this rules mgr will generate. Ex mysite.com/rules.php and what else?

Second question is that I am not getting the "rules" icon or text in my nav bar. I am using a prosilver offshoot theme. Any assistance in getting this to display would be greatly appreciated!

1. Is there a way to use BBCodes (color, bold,...) for the descriptions?

html codes work in the help_rules.php file

2. The top of the page says "Forum Rules". Where can I change this to something else?

I'm getting this error when i tried to execute the SQL query, I'm 100% that I follow the steps correctly.
My Board version is 3.0.4

SQL query:

INSERT INTO phpbb_profile_lang( rules, lang_id, lang_name, lang_explain, lang_default_value )
VALUES ( 1, 1, 'Do you agree with our forum rules?', '<a href="http://area52b.com/rules.php">Click to display them</a>', '' )

This is the Query that I have entered.
INSERT INTO phpbb_profile_lang (rules, lang_id, lang_name, lang_explain, lang_default_value) VALUES (1, 1, 'Do you agree with our forum rules?', '<a href="http://mysite/rules.php">Click to display them</a>', '')

mysite = just for testing.

Also, you may use --- to create columns just like in the FAQ from PHPBB. Look how it is set up in your MODS section in the ACP.
